Unite Canberra Care Services Pty Ltd is seeking a committed and experienced Family Support Worker to join our professional care team in Canberra, ACT.
The successful candidate will provide assessment, coordination, and goal-focused community access support to participants and their families. This role involves working closely with families, plan nominees, support coordinators, health providers, community organisations, and government agencies to ensure participants receive tailored support aligned with their NDIS goals, wellbeing needs, and independence objectives.
Key Responsibilities
- Assess participants’ needs through interviews, functional observations, and collaboration with families, plan nominees, and support coordinators.
- Develop and implement person-centred support programs to improve daily living skills, independence, and community participation.
- Monitor participant progress, maintain accurate case notes, and contribute to care plan reviews.
- Liaise with welfare agencies, health providers, government bodies, and community organisations.
- Support participants in accessing appointments, recreational programs, workshops, and skill-building activities.
- Provide practical support to families to help them navigate services and strengthen capacity.
- Prepare reports and documentation for internal reporting, funding requirements, and compliance obligations.
- Follow NDIS, WHS, incident reporting, and organisational policies.
Skills & Experience
- Minimum AQF Diploma in Community Services, Social Welfare, Human Services, Disability, Mental Health, or related discipline, or equivalent experience.
- Experience in family support, community services, disability support, casework, or related field.
- Strong understanding of person-centred practice and family-focused support models.
- Excellent communication, documentation, and liaison skills.
- Ability to assess needs, develop support plans, and monitor progress.
- Current WWVP Check or ability to obtain.
- Current National Police Check.
- Current Driver Licence and willingness to travel.
- NDIS Worker Screening Check required.
- Approved AFP check required.
